Hi,
just listening, liked it in general, missing points like:
- Risk Review with Steering Team : What is their level of risk appreciation to start / continue / terminate the Project ?
But always good to get outside views...

Werner,

Very true. Many projects descend in "troubled" status yet they don't get killed even if they should. Maybe a reason for that is lack of appreciation.

Fukushima had more than it's share of poor risk management. Putting the generators in the basement of Units 3 and 4 was a general bad idea, even if it did save money.

But the "killer" problem was that right before the wave rolled over the sea wall the operators had stopped the gravity feed cooling system because was cooling too much, which would have damaged the structure of the core. So they closed the valves.

When the wave came, it knocked out the power and of course the valve actuators were electrically powered. It went down hill from there.
